  • Tiered Statewide PPO
  • Leverages benefit design to increase utilization at cost-effective providers
  • There are three benefit tiers: 
    • Tier 1 - Blue Choice OPT PPO. Lower cost-share when this network is utilized.
    • Tier 2 - PPO Network. Higher cost-share when accessing broader PPO network.
    • Tier 3 - Out of network. Highest cost-share when out-of-network providers are used.
