We’ve updated prior authorization requirements for hospice services for members with Blue Cross Community Health PlansSM, effective Jan. 1, 2026.
What’s changing: We previously notified you that you must include a signed Certificate of Terminal illness form with your prior authorization request, effective Jan. 1, 2026. You must also include Health and Family Services Form 1592, 1593 or 1594 with your prior authorization submission. Any requests without the signed CTI and correct HFS form will be returned or denied.
Important reminders: Always check eligibility and benefits through Availity® Essentials or your preferred vendor prior to rendering services. This step will confirm prior authorization requirements and utilization management vendors, if applicable.
